The Care Binder Checklist
When everything's in one place, anyone in the family can step in on a hard day. Gather these once, keep them current, and breathe a little easier.
The essentials
- Full legal name, date of birth, and a recent photo
- Insurance cards (front & back) and Medicare/Medicaid info
- Photo ID and Social Security number (stored securely)
- Pharmacy name, phone, and account details
Medical
- Current medication list with doses and times
- Allergies and past reactions
- Diagnoses and medical history summary
- Doctors & specialists — names, phones, what they treat
- Upcoming appointments and a visit log
- Recent test results and discharge papers
Legal & financial
- Healthcare proxy / medical power of attorney
- Living will or advance directive (and DNR, if any)
- Financial power of attorney
- List of accounts, bills, and auto-payments
- Long-term care or life insurance policies
The personal side
- Daily routine and preferences (foods, sleep, comforts)
- The people and pets who matter most
- Faith, traditions, and what brings them joy
- Emergency contacts and who to call first
